Description
Grampians Integrated Cancer Service is developing a new model of care for Cancer patients in regional Victoria. The model of care will undergo a research evaluation to determine whether clinical teams with visibility to Patient Reported Outcomes (PROs) via digital pathways can enhance the care experience and health outcomes for cancer patients in regional communities.
This will support patients that may not have the luxury of seeing their care provider as often as needed due to distance, travel and COVID-related challenges in regional communities.
Join Glenn Reeves, Cancer Service Improvement Officer at Grampians Integrated Cancer Service, and Guillaume Leroux, Customer Experience Manager RN at Personify Care, as they discuss:
